Strengthening Primary Health Care Through Community Leadership

In many hard-to-reach communities across South Sudan, access to quality primary health care remains a challenge. Distance, limited resources, and weak systems often prevent families from receiving timely and essential services. At Action Aid Initiative (AAI), we believe that sustainable health outcomes begin at the community level.

Through our integrated health programmes, AAI works alongside local health facilities, community health workers, and government counterparts to strengthen service delivery and referral systems. By supporting frontline workers, improving outreach services, and embedding social and behaviour change approaches, we help ensure that communities are informed, engaged, and better able to access care.

Our approach goes beyond service provision. We focus on strengthening systems so that health services remain responsive, inclusive, and resilient long after project activities end. By placing communities at the centre of solutions, AAI contributes to improved health outcomes and stronger local ownership.
